106.01 更新(補充Bosmin)

(Infusion Rate = “Y” ml/hr, BW=body weight, assume target dose = “X” μg/kg/min)

一. 昇壓劑 ─ For shock with poor response of fluid challenge

Dobutamine(250mg/20ml/amp)

• 配法：2 amp(500mg) in D5W 500ml (即濃度1000 μg/ml)
• 劑量：Y=60X*BW/1000 
• 治療劑量：2-20 μg/kg/min

Dopamine(200mg/5ml/amp)

• 配法：2 amp(400mg) in N/S 500ml (即濃度800 μg/ml)  [或double dose：2amp in 250ml]
• 劑量：Y=60X*BW/800
• 治療劑量：up to >20 μg/kg/min

Norepinephrine(4mg/4ml/amp)

• 配法：1amp(4mg) in D5W 250ml (即濃度16 μg/ml)，但更常見 4amp in D5W 250ml的泡法。
• 劑量：一般以1 μg/min 開始滴注，titrate to desired effect，即約4 ml/hr 起始。2amp in 250的話,最高60ml/hr 或<40ml/hr
• 小心extravasation 會local necrosis

     *Catecholamines should not be given with alkaline fluid. (ex. NaHCO3)

二. 降壓劑(血管擴張) ─ For APE, Hypertensive crisis, NSTEMI/UA…etc

Nitroglycerin(5mg/10ml/amp)

• 配法：10 amp(50mg) in N/S 250ml(即濃度200 μg/ml) *Glass bottle 劑量：一般以5 μg/min開始滴注，每次增加5 μg/min，titrate to desired effect，即約1.5ml/hr起始，每次往上調1.5ml/hr。

       * Methemoglobinemia

       * Nitrate tolerance: could appear in 24hrs of drug infusion 

       → daily drug-free interval of at least 6 hours.

Nitroprusside(50mg poeder/amp)

• 配法：1 amp(50mg) in D5W 250ml (即濃度200 μg/ml)
• 劑量： Y=60X*BW/200
• 治療劑量：0.5~5 μg/kg/min

* Cyanide intoxication: behavior change,Nipride tachyphylaxis,seizure,coma…

→ should not use pulse oximetor, draw ABG(CO中毒不可依靠血氧計, 而是應該抽ABG)

→ 100% O2 + sodium nitrite(300mg IV over 15min) + sodium thiosulfate(12.5g IV

over 15min)

* Thiocyanate intoxication: anxiety,confusion,pupillary constriction,tinnitus, hallucination,seizure… → add 500mg thiosulfate for every 50mg of Nipride.

三. 其他

Bosmin(1amp/50ml for syringe pump)

• 配法：5 amp(5mg) in D5W 250ml
• 治療劑量:3-12ml/hr 用synringe pump

*Levophed 變為septic shock 第一線vasopresser。若不足者才會加上Bosmin.
Dopamine因為會造成心跳過快，所以只用於tachyarrythmia risk很低者，或absolute or relative bradycardia者。
